Replacing a Lost Registration

The best way to apply for a duplicate registration card is through MVA's convenient online service. All you need is a vehicle title number, license plate number, and form of payment (Visa, MasterCard or check). A duplicate registration card is $5.
MVA's online service lets you print a temporary registration card after you've completed the application, so try to apply on a computer that's connected to a printer. This is helpful in case you get pulled over prior to receiving your new registration card in the mail.
Replacing Lost Stickers
If you just need to replace a lost sticker for your license plate, you can download an application for duplicate stickers. Take the completed form to your local MVA office or mail it to the following address with the applicable fees ($5 for replacement stickers):

Make checks or money orders payable to "Motor Vehicle Administration" and write your driver's license number and a phone number on the check. Make sure there's a preprinted name and address on the check, or else the MVA won't accept it.
Replacing Lost Tags
MVA hasn't quite gotten to the point of issuing duplicate plates online, so you'll have to do it the old-fashioned way. Either pick up an application for duplicate plates at your local MVA office or download the application online.
Complete the application and include a check for the $20 fee (includes replacement plates and stickers). Make checks or money orders payable to "Motor Vehicle Administration." You'll need to include all relevant information: plate number, title number, insurance information, etc.
